Emergency Department Case Management Implementation

The Emergency Department is the major entry point for admissions in most hospitals – yet it is a gate left largely unprotected. Emergency Department Case Management programs have repeatedly demonstrated their value in redirecting patients to appropriate levels of care, influencing capacity, assigning correct levels of care (Outpatient Observation vs. inpatient admission), attention to the issues presented by readmissions and recommending options for safe discharge.
