Onsite Training/Coaching for Case Management Leaders and Staff

The hallmark of a contemporary and effective case management team is the knowledge, skills, and values of the staff members.  The Center offers comprehensive and up-to-date onsite training and coaching experiences, both in group and individual formats.  Competencies include the core role of case management (utilization management, transition planning, readmission avoidance, managing the patient continuum) as well as promoting skills in the area of crucial conversations, conducting family meetings, influencing change, and understanding and responding to pertinent data.

Leadership development and coaching provides your Case Management leader with the skills and tools to effectively lead a care management program and to have a voice for improvements and effective organizational outcomes.  Today, it is more important than ever to develop a Case Management leader to be a content expert, influential member of the leadership team, well versed in data, and considered solution based.

Our coaches are and have been outstanding leaders in their organizations, having created exceptional strategies around transitions in care, readmission avoidance, cost reduction, and data support.  Many of our consultants have been given progressively more responsibility as a result of their success with case management.

  • Coaching/Mentoring of Case Management Social Workers, RNs, Clinical Documentation Improvement Specialists; available per day or week
  • Coaching/Mentoring of Case Management  Leadership Staff; available per day or week
